Substantial Weight Loss Can Reduce Risk of Severe COVID 19 Complications - جراحی کاهش وزن و کووید۱۹

Successful weight-loss intervention before infection associated with 60% lower risk of severe disease in patients with obesity. A Cleveland Clinic study shows that among patients with obesity, prior weight loss achieved with bariatric surgery was associated with a 60% lower risk of developing severe complications from COVID-19 infection. The research was published in the journal JAMA Surgery.

Numerous studies have established obesity as a major risk factor for developing serious illness from an infection of SARS-CoV-2, the virus that causes COVID-19. Obesity weakens the immune system, creates a chronic inflammatory state, and increases risk for cardiovascular disease, blood clots, and lung conditions. All of these conditions can complicate COVID-19.

The aim of this study was to examine whether a successful weight-loss intervention in patients with obesity prior to contracting COVID-19 could reduce the risk of developing a severe form of this disease.

A total of 20,212 adult patients with obesity were included in this observational study. A group of 5,053 patients with a body mass index (BMI) of 35 or greater who had weight-loss surgery between 2004 and 2017 were carefully matched 1:3 to non-surgical patients, resulting in 15,159 control patients. Compared with those in the non-surgical group, patients who had bariatric surgery lost 19% more body weight prior to March 1, 2020 (the beginning of the COVID-19 outbreak in Cleveland).

After the COVID-19 outbreak, researchers looked at four COVID-19-related outcomes: rate of contracting SARS-CoV-2 infection, hospitalization, need for supplemental oxygen and severe disease (defined as a combination of ICU admission, need for mechanical ventilation or death).

Although the rate of contracting SARS-CoV-2 was similar between the groups (9.1% in the surgical group and 8.7% in the non-surgical group), participants in the weight-loss surgery group experienced much better outcomes after contracting COVID-19 compared with those in the non-surgical group. Researchers found that patients with prior weight loss surgery had a 49% lower risk of hospitalization, 63% lower risk of need for supplemental oxygen, and 60% lower risk of developing severe COVID-19.

Although the exact underlying mechanisms are not known, these data suggest that patients who underwent weight-loss surgery were healthier at the time of contracting a SARS-CoV-2 infection, which resulted in better clinical outcomes.

Telehealth-delivered diet and exercise program eased knee pain and triggered weight loss - تله مدیسین و درد زانو

New research investigating the benefits of telehealth-delivered exercise and diet programs has found 80 per cent of participants experienced improvement in knee pain and an average of 10 per cent in loss of body weight, with one man shedding 39 kilograms.

More than 400 individuals with knee osteoarthritis participated in the Better Knee, Better Me trial, developed by the University of Melbourne in partnership with Medibank and Austin Health.

Published in the Annals of Internal Medicine, the study shows researchers evaluated two six-month telehealth-delivered exercise programs, one with and one without a weight-loss dietary program, compared with an information-only control group.

During the trial, participants in the intervention groups were provided support from physiotherapists and dietitians via Zoom and a suite of resources. Those in the exercise plus diet group also received meal replacements so they could undertake a ketogenic low energy diet.

Compared to the group that only received information, both intervention programs resulted in benefits for pain, function and quality of life. Compared to the exercise-only program, the combined exercise and diet program led to additional benefits – including a greater reduction in pain, greater improvements in physical function, lower use of pain medications, and significant weight loss. After both programs, participants were also less willing to undergo knee joint replacement surgery.

Lead researcher and Director of the Centre for Health, Exercise and Sports Medicine at the University of Melbourne Professor Kim Bennell, said participants lost on average 10.2 kilograms over a six-month period with four out of five participants achieving significant improvement in pain. She said 30 per cent of participants lived in regional and rural Australia.

Obese patients over the age of 60 can lose an equivalent amount of weight as younger people using only lifestyle changes, according to a new study from the University of Warwick and University Hospitals Coventry and Warwickshire (UHCW) NHS Trust that demonstrates that age is no barrier to losing weight.

The researchers hope that their findings will help to correct prevailing societal misconceptions about the effectiveness of weight loss programmes in older people, as well dispel myths about the potential benefits of older people trying to reduce their weight.

The findings are based on analysis of patient records from a hospital-based obesity service and are reported in the journal Clinical Endocrinology.

Weight loss is most successful in heart attack survivors when partners join in the effort to diet, according to research presented today at ESC Congress 2020.

The RESPONSE-2 trial previously found that heart attack survivors referred to programmes for weight reduction, physical activity, and smoking cessation were more likely to modify behaviours compared to those receiving usual care. In both groups, living with a partner was linked with greater success in shifting bad habits. The most notable improvements were in patients who took part in lifestyle programmes and lived with a partner.

This follow-up study investigated whether partner involvement in lifestyle programmes had an impact on behaviour change. “If partners contribute to adopting healthy habits, it could become an important recommendation to avoid recurrent heart attacks,”

Transport noise is a major problem in Europe, with over 100 million people living in areas where road traffic noise exceeds levels greater than 55dB, the health-based threshold set by the EU. A new study by the University of Oxford and the University of Leicester has found a connection between traffic noise and obesity. Long-term exposure to road traffic noise, such as living near a motorway or on a busy road, was associated with an increase in body mass index and waist circumference, which are key markers of obesity. The study was published today in the journal Environmental Research.

‘While modest, the data revealed an association between those living in high traffic-noise areas and obesity, at around a 2% increase in obesity prevalence for every 10dB of added noise,’ says lead author Dr Samuel Yutong Cai, a senior epidemiologist at the University of Oxford. ‘The association persisted even when we accounted for a wide range of lifestyle factors, such as smoking, alcohol use, physical activity and diet, as well as when taking into account socio-economic status of both individuals and the overall area. Air pollution was also accounted for, especially those related to traffic.’
