In a recent phase 2b/3 clinical trial, a third mRNA vaccine against COVID-19 — known as CVnCoV and developed by CureVac — reported approximately 48 percent efficacy against symptomatic disease. In a head-to-head test of a revised version of the vaccine, CV2CoV, researchers at Beth Israel Deaconess Medical Center (BIDMC) assessed the vaccines’ ability to provoke an immune response as well as their protective efficacy against COVID-19 in non-human primates. Their findings, published in Nature, show the modifications made to the second-generation CV2nCoV induced a ten-fold higher antibody response than the original version, CVnCoV.

Air Pollution Does Not Increase the Risk of Getting Infected - آلودگی هوا و کرونا

A series of studies suggest that regions with higher pre-pandemic levels of air pollution had a higher incidence of COVID-19 cases and deaths. However, the reasons for this associations are not yet clear; air pollution could favor airborne transmission of the virus, or it could increase an individual’s susceptibility to infection or disease. “The problem is that previous studies were based on reported cases, which had been diagnosed, but missed all the asymptomatic or undiagnosed cases,” says Manolis Kogevinas, ISGlobal researcher and first author of the study.

The study included 9,605 participants among which there were 481 confirmed cases (5%). In addition, blood samples from over 4,000 participants were taken to determine the presence and quantity of IgM, IgA and IgG antibodies to five viral antigens. Of these, 18% had virus-specific antibodies, but no association was found between infection and exposure to air pollutants. However, among those who were seropositive (i.e. got infected), an association was found between higher exposure to NO2 and PM2.5 and higher levels of IgG specific for the five viral antigens (an indication of higher viral burden and/or symptom severity).

For the total study population (the 9,605 participants), an association was found between higher exposure to NO2 and PM2.5 and disease (symptoms), particularly for severe cases that ended in the hospital or in intensive care. The association with PM2.5 was stronger for men over 60 years of age and people living in socioeconomically deprived areas.

“Our study provides the strongest evidence globally on the association of ambient air pollution and COVID-19,” says Kogevinas. “These results are in line with the association between air pollution and hospitalization described for other respiratory diseases such as influenza or pneumonia”. Air pollution could also contribute by favouring the development of cardiovascular, respiratory or other chronic conditions, which in turn increase the risk of severe COVID-19.

COVID Patients on SSRI Antidepressants Are Less Likely to Die - ارتباط مصرف فلوکستین و کاهش مرگ ومیر کرونایی

A large analysis of health records from 87 health care centers across the United States found that people taking a class of antidepressants called selective serotonin reuptake inhibitors (SSRIs), particularly fluoxetine, were significantly less likely to die of COVID-19 than a matched control group.

The results add to a body of evidence indicating that SSRIs may have beneficial effects against the worst symptoms of COVID-19, although large randomized clinical trials are needed to prove this.

The UCSF-Stanford research team analyzed electronic health records from the Cerner Real World COVID-19 de-identified database, which had information from almost 500,000 patients across the U.S. This included 83,584 adult patients diagnosed with COVID-19 between January and September 2020. Of those, 3,401 patients were prescribed SSRIs.

The large size of the dataset enabled researchers to compare the outcomes of patients with COVID-19 on SSRIs to a matched set of patients with COVID-19 who were not taking them, thus teasing out the effects of age, sex, race, ethnicity, and comorbidities associated with severe COVID-19, such as diabetes and heart disease, as well as the other medications the patients were taking.

The results showed that patients taking fluoxetine were 28 percent less likely to die; those taking either fluoxetine or another SSRI called fluvoxamine were 26 percent less likely to die; and the entire group of patients taking any kind of SSRI was 8 percent less likely to die than the matched patient controls.

Though the effects are smaller than those found in recent clinical trials of new antivirals developed by Pfizer and Merck, the researchers said more treatment options are still needed to help bring the pandemic to an end. JAMA Network Open

Coronavirus disease (COVID-19) is a multifaceted infectious disease. While, at the start of the global pandemic, it was assumed that COVID-19 was primarily a disease of the lungs, it is now known that several functional systems in the human body are affected following infection with the pathogen SARS-CoV-2. One of these functional systems is blood clotting. COVID-19 patients have an increased risk of thromboses and embolisms, such as strokes, pulmonary or myocardial infarctions, and even deep vein thromboses. The use of drugs that inhibit blood clotting has been part of the treatment guidelines for COVID-19 since July 2020. “These complications during hospitalisation have a direct impact on the well-being of patients and increased the risk of dying from COVID-19,” reports David Pereyra from MedUni Vienna’s Department of General Surgery, who is first author of the publication. The underlying coagulopathy is still not fully understood.

COVID-19 triggers unique clotting problems

“The coagulopathy observed in COVID-19 patients is novel and differs in many respects from previously known coagulation problems,” says Alice Assinger, group leader at the Institute of Vascular Biology and Thrombosis Research at the Medical University of Vienna and last author of the publication, “COVID-19-associated coagulopathy displays characteristics that, although partially comparable with other coagulation diseases, cannot be fully explained by them.” Alice Assinger’s group therefore started to look for an explanation for this sub-condition of COVID-19 in the spring of 2020, in an early phase of the pandemic.

In a multi-centre analysis of COVID-19 patients in Vienna, Linz and Innsbruck, the group observed that COVID-19-associated coagulopathy occurs almost exclusively in patients requiring intensive care or in patients who die as a result of COVID-19. Although anticoagulant drugs improve the survival of COVID-19 patients, they show no effect on immunological processes related to blood coagulation (immunothrombosis).

Low-molecular-weight heparin curtails duration of infection

The analyses showed, however, that the period of active SARS-CoV-2 infection is curtailed in patients treated with low-molecular-weight heparin, the most commonly used anticoagulant. “In patients who receive this drug, infection time is an average of four days shorter than in patients who are not treated with low-molecular-weight heparin. We were surprised to see that low-molecular-weight heparin may have a direct effect on coronavirus and its infectivity,” said David Pereyra. In short, a lot of people suddenly became more sedentary during the onset of the pandemic. Recently published research found people who continued to spend a higher amount of time sitting between April and June 2020 were likely to have higher symptoms of depression. A closer investigation into this association could play a role in helping people improve their mental health.

To get a snapshot of those changes, Meyer and a team of researchers received survey responses from more than 3,000 study participants from all 50 states and the District of Colombia. Participants self-reported how much time they spent doing activities, like sitting, looking at screens and exercising, and how those behaviors compared to pre-pandemic times. Using standard clinical scales, they also indicated changes to their mental wellbeing (e.g., depression, anxiety, feeling stressed, lonely).

This paper in Frontiers in Psychiatry served as a follow up to see whether the participants’ behaviors and mental health changed over time. Participants filled out the same survey each week between April and June.

This study recommended people take breaks when sitting for long periods of time.

Study finds the SARS-CoV-2 virus can infect the inner ear - حمله کرونا به گوش داخلی

The prevalence of auditory symptoms in Covid-19 patients is unknown, but infection of the inner ears may be responsible for hearing and balance problems.

Many Covid-19 patients have reported symptoms affecting the ears, including hearing loss and tinnitus. Dizziness and balance problems can also occur, suggesting that the SARS-CoV-2 virus may be able to infect the inner ear.

A new study from MIT and Massachusetts Eye and Ear provides evidence that the virus can indeed infect cells of the inner ear, including hair cells, which are critical for both hearing and balance. The researchers also found that the pattern of infection seen in human inner ear tissue is consistent with the symptoms seen in a study of 10 Covid-19 patients who reported a variety of ear-related symptoms.

The researchers used novel cellular models of the human inner ear that they developed, as well as hard-to-obtain adult human inner ear tissue, for their studies. The limited availability of such tissue has hindered previous studies of Covid-19 and other viruses that can cause hearing loss.

The study and results are published in the Journal of Communications Medicine.

Researchers from the CDC’s VISION Network gathered data from more than 201,000 hospitalizations in nine different states. About 7,000 people in that group fit the criteria for this study. The research team analyzed the number of unvaccinated individuals who had a positive COVID-19 test more than three months before being hospitalized for the virus as well as the number of individuals who received the Pfizer or Moderna vaccine and were not diagnosed with COVID prior to being admitted to the hospital. The research team found that overall, unvaccinated adults with a previous COVID-19 infection were about five times more likely to be hospitalized than those who were vaccinated.

As SARS-CoV-2 continues to spread and mutate, it’s important to identify new treatment options. Molnupiravir is an investigational oral antiviral being developed for the treatment of COVID-19, and has been submitted for review by the U.S. Food and Drug Administration. Researchers now report in ACS Central Science that they have engineered enzymes to help manufacture the pill, resulting in a much shorter and higher-yielding synthesis than current methods.

Originally developed to treat influenza, molnupiravir works by causing viruses to make errors when copying their own RNA, introducing mutations that inhibit replication. Recently, interim phase 3 clinical trial findings indicated that molnupiravir reduced the risk of hospitalization and death from COVID-19 for newly diagnosed, at-risk patients, and that it worked equally well against different SARS-CoV-2 variants. John McIntosh, Patrick Fier and colleagues at Merck and Codexis set out to develop a shorter, higher-yielding and sustainable way to synthesize the molecule.

The researchers developed a three-step synthesis of molnupiravir from a sugar molecule called ribose. They identified enzymes or chemical treatments to sequentially add the appropriate chemical groups to ribose to generate the molecule. For the second step of the synthesis, the team identified bacterial enzymes that weakly catalyzed the desired reactions. Using in vitro evolution, they greatly enhanced these enzymes’ activities. The new synthetic route, which also included a phosphate recycling strategy, was 70% shorter and had a seven-fold higher overall yield than the original route.

Statins are a recommended and common intervention for preventing cardiovascular events by reducing levels of lipoprotein cholesterol in the blood. During the pandemic, it has been debated whether statins influence the risk of death from COVID-19. Researchers at Karolinska Institutet have now conducted the largest population study to date in the field. The study, which is published in PLOS Medicine, indicates that statin treatment slightly lowers COVID-19 mortality.

Statins are used to lower the cholesterol level – the lipid count – in the blood and are a common preventative treatment in patients at high risk of cardiovascular events.

During the pandemic, the question of whether statins can reduce COVID-19 mortality via their effects on coagulation and the immune system has engaged scientists and doctors.

Earlier studies have not provided an unequivocal answer and have often suffered from the limitation that they have only included hospital inpatients. Researchers at Karolinska Institutet have now carried out the largest population study to date on the relationship between statins and COVID-19 mortality.

More than half of the 236 million people who have been diagnosed with COVID-19 worldwide since December 2019 will experience post-COVID symptoms — more commonly known as “long COVID” — up to six months after recovering, according to Penn State College of Medicine researchers. The research team said that governments, health care organizations and public health professionals should prepare for the large number of COVID-19 survivors who will need care for a variety of psychological and physical symptoms.

During their illnesses, many patients with COVID-19 experience symptoms, such as tiredness, difficulty breathing, chest pain, sore joints and loss of taste or smell.

Until recently, few studies have evaluated patients’ health after recovering from the coronavirus. To better understand the short- and long-term health effects of the virus, the researchers examined worldwide studies involving unvaccinated patients who recovered from COVID-19. According to the findings, adults, as well as children, can experience several adverse health issues for six months or longer after recovering from COVID-19.

The researchers conducted a systematic review of 57 reports that included data from 250,351 unvaccinated adults and children who were diagnosed with COVID-19 from December 2019 through March 2021. Among those studied, 79% were hospitalized, and most patients (79%) lived in high-income countries. Patients’ median age was 54, and the majority of individuals (56%) were male.

The researchers analyzed patients’ health post-COVID during three intervals at one month (short-term), two to five months (intermediate-term) and six or more months (long-term).

According to the findings, survivors experienced an array of residual health issues associated with COVID-19. Generally, these complications affected a patient’s general well-being, their mobility or organ systems. Overall, one in two survivors experienced long-term COVID manifestations. The rates remained largely constant from one month through six or more months after their initial illness.

The investigators noted several trends among survivors, such as:

  • General well-being: More than half of all patients reported weight loss, fatigue, fever or pain.
  • Mobility: Roughly one in five survivors experienced a decrease in mobility.
  • Neurologic concerns: Nearly one in four survivors experienced difficulty concentrating.
  • Mental health disorders: Nearly one in three patients were diagnosed with generalized anxiety disorders.
  • Lung abnormalities: Six in ten survivors had chest imaging abnormality and more than a quarter of patients had difficulty breathing.
  • Cardiovascular issues: Chest pain and palpitations were among the commonly reported conditions.
  • Skin conditions: Nearly one in five patients experienced hair loss or rashes.
  • Digestive issues: Stomach pain, lack of appetite, diarrhea and vomiting were among the commonly reported conditions.
